Getting started with Advantage Physical Therapy is extremely easy!

Call our office to make an appointment at 516-374-5310 or fill out the form below. Set aside a time slot of an hour for your first visit, as it will include a complete initial evaluation and treatment will be initiated.

Is a Perscription required?
We are able to see you with or without a prescription to get you started with your rehabilitation plan.
However due to Medicare guidelines patients with a Medicare policy will need a script to receive care.
COME PREPARED
On your first visit, please fill out all applicable forms and bring:
Your insurance card
A driver’s license or other picture ID
Comfortable clothing and shoes
Payment for a deductible or co-payment if applicable (we accept all credit cards, personal checks, and cash)
